Managing 340B rebates across multiple TPAs

Many covered entities split their contract pharmacy network across more than one 340B TPA. Starting January 1, 2027, the HRSA 340B Rebate Model Pilot Program has covered entities buy selected drugs at a non-340B price, submit claims data to Beacon, and receive the 340B discount as a rebate. With several TPAs, that means several sources of rebate claims, several sets of deadlines and several places to look for payments. Aventi Health is an independent 340B TPA built for the rebate era, and rebate submission works whether or not Aventi is your only TPA.

The problem

Why multiple TPAs make 340B rebates harder

  • Scattered claims data

Each TPA holds the dispense data for its own pharmacies, so rebate-eligible claims for the same drug can live in different systems.

  • Different deadlines to track

Claims data is due to Beacon within 45 calendar days of the date of dispense, no matter which TPA or pharmacy the claim came from.

  • Duplicate risk

When more than one system could submit the same claim, a covered entity needs a clear rule for which claims go to Beacon and from where.

  • Payments with no single view

Manufacturer rebate payments and denials have to be matched back to claims, which is slow when the claims sit with different vendors.

Rebate model basics

What the 2027 rebate model requires, regardless of TPA

  • Submit within 45 days

Covered entities submit claims data to Beacon within 45 calendar days of the date of dispense, with exceptions for extenuating circumstances and 340B status changes.

  • Paid or denied in 10 days

Manufacturers must pay or deny, with documentation, each rebate within 10 calendar days of a complete data submission.

  • Rebate equals WAC minus ceiling price

The rebate is the wholesale acquisition cost (WAC) minus the 340B ceiling price on the date of dispense, so each claim needs accurate drug, quantity and date data.

A better setup

How to run 340B rebates when you have more than one TPA

  1. Step 1: Map every pharmacy to its data source

List each contract pharmacy and in-house pharmacy, which TPA holds its data, and which selected drugs it dispenses.

  2. Step 2: Pick one submission owner per claim

Decide which system submits rebate claims to Beacon for each pharmacy, so every eligible claim is submitted once and only once.

  3. Step 3: Use one place to reconcile

Match manufacturer payments, short pays and denials against submitted claims in one view instead of in a different portal per TPA.

  4. Step 4: Review aging every week

Watch for claims nearing the 45-day submission window and rebates still unpaid after 10 days, then follow up with the manufacturer and Beacon.

How Aventi helps

One rebate workflow, even with multiple TPAs

  • Rebates without switching TPAs

Aventi's rebate claim submission works even if Aventi is not your TPA, so you can keep existing TPA relationships while rebates run in one place.

  • Conforming claims for Beacon

Aventi prepares pharmacy and medical rebate claims for Beacon with conforming claim units, so submissions aren't returned as incomplete.

  • Reconciliation and denial tracking

Paid, short-paid, unpaid and denied rebates are tracked by manufacturer and pharmacy, so nothing falls between two vendors.

For pharmacies where Aventi is the TPA, rebate claims live in the same system of record as contract pharmacy and split billing, so there are no end-of-month true-ups.

Specialty pharmacy

Specialty pharmacy rebates across TPAs

Specialty and infusion drugs are often the highest-value rebates at stake, and specialty pharmacies are often the ones split across different TPAs. Aventi has exclusive integrations and a gateway with BioPlus, Accredo and Optum, plus many other specialty pharmacies, so specialty dispense data reaches rebate claims on time and complete.

FAQ

340B rebates with multiple TPAs FAQ

Yes. Many covered entities already use different TPAs for different contract pharmacies. Under the 340B Rebate Model Pilot Program, claims data for selected drugs still has to reach Beacon within 45 calendar days of the date of dispense, whichever TPA holds the data.

Assign one submission owner for each pharmacy's rebate claims and keep a single list of submitted claims. A rebate already paid for the same claim is one of the reasons a manufacturer can deny with documentation.
