Aventi vs SunRx: choosing between a PBM-owned 340B TPA and an independent one

SunRx is a San Diego-based 340B administrator that has managed 340B programs since 2006. It says it is a wholly-owned subsidiary of MedImpact Healthcare Systems, a pharmacy benefit manager.[1][2] That makes SunRx different from TPAs owned by a wholesaler or a pharmacy chain: its parent's core business is managing pharmacy benefits for health plans and employers. This page compares SunRx and Aventi on what matters to a covered entity, covers what MedImpact's 2026 deals mean for SunRx clients, and lists the questions to ask before you renew.

Aventi Health is an independent, founder-owned 340B TPA serving 300 covered entities, 24 of them hospitals.

Ownership

One parent, both sides of the pharmacy benefit

SunRx works for covered entities. Its parent, MedImpact, works for the plans that pay pharmacy claims.

  • Your 340B program

    SunRx

Administers 340B for hospitals, health centers and their contract pharmacies: eligibility, contract pharmacy networks, split billing, referral capture and an uninsured 340B prescription discount card.[1][3]

  • Health plans and employers

    MedImpact

Serves employers and commercial and government health plans covering more than 20 million consumers, including hospital-based employers. MedImpact says its 340B solutions are offered through SunRx.[11]

SunRx presents the PBM connection as a strength. It says its uninsured 340B prescription discount card is unique in the market because SunRx is owned by a PBM, and that it confirms 340B eligibility in real time at the pharmacy and applies a "lower-of" price calculation for the patient.[2][18] MedImpact says the SunRx team has extensive pharmacy benefit management and payer experience.[11]

We are not aware of any public allegation that MedImpact or SunRx has misused 340B data. Still, when one company is both your 340B administrator and a PBM for health plans, it is reasonable to ask how the two businesses are separated, especially if your hospital also uses MedImpact as its employee pharmacy benefit manager.

Your TPA's parent

MedImpact in 2026: a growing PBM group

Here is what SunRx's parent has announced since 2024, from public sources.

  1. February 2024

MedImpact acquired Elixir Solutions, Rite Aid's PBM.[13]

  2. February 2, 2026

MedImpact completed its acquisition of Sav-Rx, a PBM serving organized labor.[12]

  3. March 2026

MedImpact acquired risk management firms MHW Benefit Partners and MSL Captive Solutions.[13]

  4. October 26, 2026

MedImpact agreed to acquire VytlOne's PBM business and the Gateway Health Partners rebate aggregator from Maxor. VytlOne's 340B, specialty and fulfillment assets are not part of the deal.[13][14]

  5. November 3, 2026

The Oregon Health Authority accepted the notice for the Maxor deal and began its review. It lists 340B program services among MedImpact's lines of business.[14]

None of these deals changes SunRx's services, as far as public sources show. For a covered entity, the practical point is that SunRx's priorities are set inside a PBM group that is focused on plans, employers and rebate aggregation, and that is still adding companies.[12][13]

Data security is part of the same picture. MedImpact has published a notice about a cybersecurity incident it identified on October 18, 2025, involving information about members of pharmacy benefit plans administered by MedImpact and Elixir Solutions. It notified clients on August 13, 2026 and began mailing notices to individuals on September 25, 2026.[15] The notice does not mention SunRx or 340B data. It is still fair to ask SunRx which systems, staff and security controls it shares with its parent.

Before you renew

Five questions to ask SunRx in writing

  • Is our 340B claims and eligibility data kept separate from MedImpact's PBM operations, and who at MedImpact can access it?
  • If MedImpact is also the PBM for our employee health plan, how are those two relationships kept apart?
  • Which systems, hosting environments and security teams does SunRx share with MedImpact?
  • Is Command340B (Assist340B, Edge340B and the reporting modules) included in our TPA fees, or priced separately?
  • If we leave, what data do we get back (claims, accumulators, CDM-to-NDC crosswalk, discount card records, 340B ESP and Beacon submissions), in what format, and how fast?

Aventi has no PBM, wholesaler or pharmacy chain on the other side of your claims. You see every claim in the same system we use to qualify it.

SunRx's services

What SunRx provides beyond claims qualification

If you are weighing a switch, note which of these you rely on so nothing falls through the cracks.

  • Hospital association partnerships

SunRx says it is a corporate partner of 340B Health and is endorsed by state hospital associations as the exclusive 340B administrator for their members, after a review of its technology and services.[1][9]

  • Pharmacy Department

A dedicated team identifies and enrolls pharmacies, negotiates dispensing fees, handles contracting and answers questions during HRSA registration. Its network includes chain, grocery, mass merchant, independent, specialty and mail order pharmacies.[3]

  • Uninsured discount card

Staff issue a card through the SunRx system; the contract pharmacy adjudicates the 340B claim in real time, and covered entities can set optional subsidy tiers for low-income and cash-paying patients.[18]

  • Advanced Claims Capture

Built with AuthorityRx, which SunRx acquired in September 2023. It reviews fallout and outside-referral claims, and SunRx says it works with any TPA and needs no EHR integration.[7][19]

  • Account team and audits

Each client gets a dedicated account executive or manager, monthly program review calls, help with policies and procedures, and HRSA audit support on request.[10]

  • Command340B

A 2026 suite covering manufacturer submissions to 340B ESP and Truzo, Beacon rebate submissions, rebate data management, transparency reporting, compliance and a pharmacy gateway.[5]

2027 rebate model

SunRx and the 340B rebate pilot: a fair read

HRSA's 340B Rebate Model Pilot starts January 1, 2027. Ten manufacturers have approved plans, all using the Beacon platform. Covered entities keep buying selected drugs through their 340B wholesaler account, submit claims data within 45 calendar days of dispense, and manufacturers must pay or deny within 10 calendar days of a complete submission.[16]

Unlike some large TPAs, SunRx has published how it plans to handle Beacon rebate submissions. Its Command340B materials say Assist340B automates Beacon rebate submissions and flags denied rebate attempts for resubmission, and Edge340B processes returned Beacon files and supports dual workflows for traditional discount and rebate contracts.[5][6] SunRx also says Assist340B data can be shared with other 340B TPAs.[6]

Two things are worth checking. First, Command340B is a set of separate modules, and SunRx does not publish whether they are included in TPA fees. Second, SunRx's "Rebate Model Myths — and Facts!" article was written while the earlier rebate model was paused and refers to nine manufacturers,[8] so confirm SunRx's workflow against the ten approved plans HRSA lists now, including medical claims for clinic-administered drugs.[16]

Aventi qualifies rebate claims, submits them to Beacon and reconciles payments in the same system that runs your eligibility, split billing and contract pharmacy claims, and the rebate module is included for Aventi TPA clients. Read about the 340B rebate model, handling rebate denials and rebate reconciliation.

Your options

Three paths for SunRx clients

You do not have to decide everything at once. Each path keeps 2027 rebate claims covered.

  • Keep SunRx, compare rebate options

If SunRx works for you, get its Command340B pricing in writing and compare it with naming Aventi your designated third party for 2027 rebate claims. Contact us for pricing. No TPA switch required.

    How Aventi handles rebate claims

  • Run two TPAs during a transition

Move some sites or contract pharmacies to Aventi first, keep others on SunRx, and keep one rebate submission and reconciliation flow across both.

    Managing rebates across multiple TPAs

  • Move your TPA services to Aventi

Bring contract pharmacy and split billing to an independent TPA with contingency-based pricing and no PBM parent. The rebate module is included for Aventi TPA clients.

Switching

Switching from SunRx to Aventi: a checklist

  1. Step 1: Check your agreement's term and notice

Find your SunRx agreement's renewal date and termination notice period, and any separate agreements for Advanced Claims Capture or Command340B modules. Review them with your counsel; this page is not legal advice.[5][7]

  2. Step 2: Export your data from the SunRx portals

Download claims, eligibility settings, accumulators, invoices and reports from the SunRx Covered Entity and Client portals before access ends.[1]

  3. Step 3: Take your CDM-to-NDC crosswalk

SunRx says it builds and maintains the CDM-to-NDC crosswalk for split billing clients. Ask for the current version so your new split billing setup starts from it.[4]

  4. Step 4: Plan the discount card transition

List patients using the SunRx 340B prescription discount card, their subsidy tiers and formulary, and set up the replacement before cards stop working at the pharmacy.[18]

  5. Step 5: Confirm pharmacy contracts and dispensing fees

SunRx's Pharmacy Department negotiates dispensing fees and contracts. Confirm which agreements are between you and each pharmacy, and tell every pharmacy the change date.[3]

  6. Step 6: Re-point 340B ESP, Truzo and Beacon

Move manufacturer data feeds that Assist340B handles today, and set your Beacon third-party designation before January 1, 2027 so rebate claims are neither missed nor sent twice.[6][16]

Aventi runs onboarding with you and can submit rebate claims while you are still on SunRx. See the demo to walk through it.

FAQ

SunRx vs Aventi: frequently asked questions

SunRx says it is a wholly-owned subsidiary of MedImpact Healthcare Systems, a pharmacy benefit manager (PBM). Its legal name in its site footer is 340B Holdings, LLC (SunRx). SunRx is based in San Diego and says it has managed 340B programs since 2006. Aventi Health is independent and founder-owned, with no PBM, wholesaler or pharmacy chain parent.

MedImpact describes itself as the nation's largest independent pharmacy benefit and health solutions company, privately held and not owned by insurers, pharmacy chains or Wall Street investors. Independent here means independent of those companies; MedImpact is still a PBM. The Oregon Health Authority describes it as the fifth largest PBM in the nation, handling about 5% of U.S. prescriptions.

Yes. The company writes its name both ways on its own website. Both refer to the San Diego-based 340B administrator owned by MedImpact.

SunRx publicly describes Beacon rebate support. Its Command340B suite includes Assist340B, which it says automates Beacon rebate submissions and uploads to 340B ESP and Truzo, and Edge340B, which it calls a Rebate Model TPA that processes returned Beacon files and supports dual workflows for discount and rebate contracts. Ask SunRx whether Command340B is part of your TPA fees or priced separately, since its fees are not published.

340B TPAs that covered entities commonly compare with SunRx include Aventi Health, Verity Solutions, Macro Helix, Wellpartner, PharmaForce, NuvemRx, Sentry Data Systems (Craneware) and Pillr Health. Aventi is the independent, founder-owned option, serving 300 covered entities, 24 of them hospitals. See aventihealth.com/340b-tpa-comparison for a sourced side-by-side.

Yes. You do not have to switch TPAs to use Aventi for 2027 rebate claim submission. Aventi can act as your designated third party for rebate claims, in-house pharmacy medical claims submissions to Beacon, and 340B ESP and Truzo claims data submissions. Contact us for pricing.

SunRx's 340B prescription discount card is issued through the SunRx system and adjudicated in real time at the contract pharmacy, and SunRx says it is unique in the market because SunRx is owned by a PBM. Before you switch, document your card program's patient tiers, copays and formulary so you can rebuild your sliding fee or subsidy program with your new TPA and pharmacies without interrupting patients.

Aventi's TPA services for contract pharmacy and split billing are contingency-based, with no true-ups, and the rebate module is included for Aventi TPA clients. SunRx does not publish its TPA fees. It does say its Advanced Claims Capture service charges no fee unless additional claims are captured. Ask each TPA for a written proposal based on your own volume.
