340B rebate reconciliation: match every manufacturer rebate payment to the claims you submitted

Under the 2027 340B rebate model, covered entities buy at a non-340B price, submit pharmacy and medical rebate claims to Beacon, and wait for manufacturers to pay the 340B discount back. Every rebate then has to be reconciled against what you submitted. Aventi Health is an independent 340B TPA built for the rebate era: rebate submissions and reconciliation happen inside the same system of record as your contract pharmacy and split billing data.

Reconciliation

What is 340B rebate reconciliation?

Reconciliation is the process of matching each manufacturer rebate payment to the rebate claims you submitted, confirming the amount paid equals the 340B discount owed, and following up on anything denied, short-paid or unpaid. Under the HRSA 340B Rebate Model Pilot Program starting January 1, 2027, claims data is submitted to Beacon within 45 days of dispense, and rebates arrive afterward, so reconciliation becomes an ongoing finance task rather than an occasional one.

Payment matching

Reconciling manufacturer rebate payments against submitted claims

  • Claim-level matching

Each submitted rebate claim is tracked through submission, acceptance and payment, so you see which claims were paid.

  • Paid vs. expected

Compare what manufacturers paid to what was expected on the claims you submitted.

  • Monthly statements

Track rebates and monthly statements once claims clear.

Follow-up

Short pays, denials and unpaid rebates

  • Short pays

Flag payments that come in below the expected rebate so your team can follow up.

  • Denials

Track denied rebate claims with their status so they can be corrected and resubmitted where allowed.

  • Good Faith Inquiries

GFI tracking in one view, from pending to approved.

Outstanding rebates

Rebate aging: knowing what is still outstanding

Aging views show how long submitted rebate claims have been waiting for payment, grouped by manufacturer and pharmacy, so finance teams know what cash is outstanding and which manufacturers to follow up with.

One platform

No true-ups: why the system of record matters

When rebate claims, contract pharmacy claims and split billing purchases live in one platform, reconciliation is not a spreadsheet exercise. Aventi ties every purchase to an eligible dispense record in real time, so there are no end-of-month true-ups, and rebate reconciliation sits alongside MFP reconciliation tools, negative balance management and duplicate discount monitoring.

Specialty pharmacy

Specialty pharmacy rebates

Specialty and infusion drugs are often the highest-value claims to reconcile. Aventi has exclusive integrations and a gateway with BioPlus, Accredo and Optum, plus many other specialty pharmacies, so specialty dispense data feeds rebate claims and reconciliation instead of arriving late or by hand.

Under the HRSA 340B Rebate Model Pilot Program, starting January 1, 2027, covered entities pay a non-340B price at purchase and receive the 340B discount later as a rebate. Every rebate has to be checked against submitted claims, so reconciliation becomes a recurring finance task.

No. Reconciliation does not disappear under the rebate model; it shifts to tracking rebate payments, denials and short pays alongside existing work such as Good Faith Inquiries and MFP reconciliation.

A short pay is a rebate payment that is lower than the amount expected for the claims submitted. Aventi flags short pays so your team can follow up with the manufacturer.

Aventi tracks how long submitted rebate claims have been outstanding, by manufacturer and pharmacy, so finance teams can see unpaid rebates and prioritize follow-up.

No end-of-month true-ups. Aventi ties every purchase to an eligible dispense record in real time and keeps rebate claims in the same system of record as contract pharmacy and split billing.
